The Medicaid Population: Identifying, Locating and Engaging a Hard-to-Reach Population for Disease Management Success, a July
25, 2007 audio conference on CD-ROM, provides proven ways to identify, locate and engage Medicaid patients in need of disease management and the results this can yield.
Listen to pre-conference comments from Dr. Bonaparte and Jacobi..
One of the greatest challenges organizations face when administering disease management programs for the Medicaid
population is locating members and initiating contact with them. Compounded by what is often outdated information
from state agencies with the transient nature of the Medicaid population, initial outreach to this population is difficult at
best. Once identified, the next step in an effective Medicaid disease management program is a set of engagement strategies used in a
variety of scientific and human service ways to effectively engage clients and get them involved in the program.
During this 90-minute audio conference on CD-ROM, industry experts Dr. Philip Bonaparte, Horizon NJ Health, and Caryn
Jacobi, associate vice president of operations for Illinois, McKesson Health Solutions, described how their
respective organizations target and engage Medicaid enrollees for participation and success in disease management programs.
Here's what participants said about the live program:
The conference provided "new tips on how to improve disease management programs," said Michelle Debernard, disease management coordinator with HealthPlus of Michigan
"Excellent use of resources and ideas on engaging, identifying and improving interventions for asthma and maternity," said a disease management specialist with a mid-atlantic health plan.
"A lot of 'out-of-the-box' thinking examples," said one participant.

- Using a multi-channel approach to identify and engage Medicaid populations;
- Utilizing engagement strategies that are most effective with the Medicaid population; and
