Description

From a patient’s or caregiver’s clear understanding of hospital discharge instructions to a phone call a few days post discharge, the hospital discharge is ripe with opportunity to ensure timely follow-up to care and patient compliance, which in turn can reduce avoidable hospital readmissions related to a patient’s failure to comply with the hospital discharge plan.

During A Coordinated Discharge Planning Approach to Reduce Avoidable Hospital Readmission, a 45-minute webinar on April 28, 2010 now available as a CD-ROM, via On Demand Web access or as a training DVD. Susan Shepard, director of patient safety education, The Doctors Management Company, broke down the hospital discharge process from the time the person is admitted to the hospital to after they are discharged and identify the key areas that can have an impact on avoidable hospital readmissions.

Ms. Shepard will:

  • Identify two areas where care transition errors occur;
  • Name two best practices for discharge process improvement;
  • Describe how to improve patient education;
  • Show how to identify the patients most at-risk for readmission; and
  • Help participants understand cost implications for poorly designed discharge processes.
