Discharge Planning Primer: Community Collaborations to Decrease Hospital Readmissions Risk

Coordinated planning of a patient's care following a hospital or nursing home stay can greatly affect health outcomes, likelihood of readmission and/or emergency room visits, as well as cost to patients, providers and insurers. A discharge management plan that integrates community resources and programs can further ease the transition from hospital to home and improve continuity of care.

Discharge Planning Primer: Community Collaborations to Decrease Hospital Readmissions Risk profiles two aptly named discharge management efforts that access and maximize partner resources for their populations. CHOICES is a hospital-based case management program for older adults in Albany, N.Y., while CASA (Community Alternative Systems Agency) in Broome County, N.Y. is a community-based initiative that collaborates with hospitals and nursing homes to help frail elders and young disabled adults. Both are client-centered models in discharge planning designed to meet the physical and psycho-social needs of their respective populations.

A Conversation With...

Dr. Lonnie E. Fuller, medical director for the Pennsylvania Medicaid ACCESS Plus Primary Care Case Management and Disease Management (PCCM-DM) program, explains how the solid relationships that patients build with ACCESS Plus providers and office staff, many of whom live in the neighborhood, keep them coming back in for care. He also discusses how "road shows" put on by ACCESS Plus's Regional Advisory Committee foster dialogue among consumers, providers and community organizations and provide a running issues list for the organization. In addition, Dr. Fuller notes that disease management and telephone triage line efforts are reducing unnecessary trips to the ER.

Each week, a healthcare professional responds to a reader's query on an industry issue. This week's expert is Roger Reed, executive vice president and chief health officer at Gordian Health Solutions.

Question: What is the role of health coaching in CDHPs?

Response:(Roger Reed) When CDHPs were first offered, only about 10 percent of employees enrolled in them. Today, about 56 percent of employees will enroll, with participation sometimes as high as 90 percent. The individual is being asked to take more responsibility, to set their own goals and to make informed decisions about their healthcare.

A recent Hewitt survey found that 93 percent of employees were comfortable with taking greater responsibility for their healthcare decisions and would be comfortable modifying their health plan. However, most of them had no idea how to accomplish it. These employees are asking how to become a better healthcare consumer and how to take responsibility and improve their own healthcare to avoid dipping into their HSAs to pay these bills.

Health coaching supports employers as they move toward consumer-driven healthcare. Health coaching also supports employees as they try to become more judicious users of healthcare and more active consumers so that they can reach their own health potential.
