ABOUT OUR PANELIST:

Jan Van der Mei
Sutter Health's case managers play a key role in helping patients understand their goals of care.

Jan Van der Mei, R.N., M.S., A.C.M., currently serves as regional director of Sutter Health Sacramento Sierra Region’s care management programs, supporting four hospitals with 800+ average daily census, two medical groups and one multi-specialty IPA.

As case management director, she is responsible for planning and operations for disease management, including heart failure, asthma, diabetes, heart healthy program with a focus on hypertension and lipid management, and care coordination/medical office case management for patients with chronic illness. She is also responsible for the anticoagulation management program.

Van der Mei has over 30 years of progressive experience in healthcare leadership in an integrated delivery system. Her broad-based experience includes nursing, case management, disease management, and administration.

In addition to serving Sutter Health as an internal consultant for system improvement and case management models, Van der Mei speaks to national audiences on case management and disease and population management issues.

Van der Mei was a founding member of the Northern California chapter of the American Case Management Association and served as its president from June 2005 through June 2007. She is also a member of the American Case Management Association and is a member of its chapter Advisory Council and the Franklin Award Committee. Van der Mei received an associate’s degree in Nursing at Pacific Union College.

She completed her Bachelor of Science in Nursing in Public Health at California State University, Dominguez Hills and received a Master of Science in Case Management from Breyer State University. Van der Mei is an accredited case manager.
