Rebecca Ramsay
CareOregon uses two techniques to identify Medicare patients for a transitional care program.

Rebecca Ramsay, B.S.N., M.P.H., is the senior manager of care support and clinical programs at CareOregon. During her five year tenure at CareOregon she has worked to develop and implement an innovative, nationally recognized team-based care management program that targets the plan’s highest risk and costliest members. Unlike many care management programs that focus outreach and management to specific disease states, CareOregon’s care support program adheres to a holistic biopsychosocial model that is rooted in population management strategies.

Over the past two years, Ramsay has worked with colleagues at Oregon Health Sciences University (OHSU) to develop a competency based motivational interviewing training program for CareOregon’s case managers. This training has profoundly enhanced the program’s client-centeredness and the case managers’ skills in self- management support for the plan’s members. Most recently, Ramsay has been developing and facilitating a primary care-based care management learning collaborative as a critical component of the medical home transformation project that CareOregon is sponsoring in partnership with their safety net providers.

After receiving her BSN, Ramsay worked for 10 years as a nurse and research associate at OHSU, with a clinical focus on promotion of self-management among women experiencing high-risk pregnancies, ambulatory women’s health, and labor and delivery. Her interest in public health principles, including population-based promotion and prevention strategies, led her to pursue an MPH.

Ramsay received her Bachelor of Science in Nursing at the University of Madison, Wis., and her Masters in Public Health at Portland State University.
