    After identifying a need in the state of Iowa for access to affordable prescription drugs and access to specialists for underserved populations, as well as medical home development for the safety net providers in the state, the Iowa/Nebraska Primary Care Association was awarded state funding to create and manage a provider network to bring together all of the community resources needed to make a medical home possible.

    During this 45-minute webinar, Ms. Dixon Gale and Ms. Stephan will share how the primary care providers in this community work with other local, community-based organizations, as part of a virtual medical home, to improve access to and the quality of care. You will get details on:

    • Developing a community utility approach to medical home development, where local public health and social service organizations can more easily provide support to primary care providers working to become medical homes;
    • How minimal amounts of funding can encourage local entities to come together to begin to develop virtual or community-based medical homes;
    • Creating formal partnerships between primary care providers and local hospitals to increase the range of services available to underserved patients and decrease the amount of uncompensated care and inappropriate use of the emergency room;
    • Using a centralized care coordinator or patient navigator to assist patients in securing necessary resources that may not be available within a primary care provider office; and
    • A case study on how a local primary care provider works with two local hospitals to coordinate the care of patients, including specialist care.

    Presenters: Laurel Karabatsos, deputy Medicaid director and Jerry Smallwood, Medicaid reform unit manager.

    With a January 2011 go-live date for an accountable care organization pilot, the Colorado Department of Health Care Policy and Financing is entering into the final stages of an RFP process to identify regional organizations that will function as ACOs, the medical homes that will serve as providers within the ACO and a state-wide data and analytics vendor that will provide real-time data to the providers within the ACO.

    During this 45-minute webinar, Ms. Karabatsos and Mr. Smallwood will walk us through the ACO development process in Colorado. From the practical challenges to the processes for addressing these challenges, they will cover:

    • The system changes required before launching an ACO;
    • Developing performance measures and an incentives program for year one of the pilot and how these will differ in the expansion year;
    • Balancing between prescriptive and permissive in enlisting healthcare providers to be accountable to their outcomes;
    • Determining provider payments and payment levels to support an ACO;
    • Specific strategies for gaining stakeholder support early in the process;
    • Identifying and addressing marketplace resistance; and
    • Creating the ACO roll-out plan.
